NOTE: Please bring photocopies and originals of all supporting documents. Original documents will be returned to applicant.

Applications with insufficient funds will be returned to the applicant, please be aware mailing fee will be deducted from the amount submitted

Please note that the Consulate General, Miami,  is responsible for processing applications received from persons resident in one of the following States:  Alabama, Arizona, Arkansas, Florida, Georgia, Louisiana, Mississippi, New Mexico, North Carolina, Oklahoma, South Carolina, Tennessee and Texas. Applications from persons resident in other States will be returned to the applicant, please be aware mailing fee will be deducted from the amount submitted

Birth, Marriage and Death Certificates from Jamaica are to be in the NEW FORMAT issued by the Registrar General’s Department ( www.rgd.gov.jm)

All first time applicants must visit the Consulate General to be interviewed by a Consular Officer and submit the following documents:

  • A Completed passport application form
  • Two (2) identical non digitized color photographs, one of which should be certified by a Notary Public who signs Section G
  • Birth Certificate (for persons born in Jamaica)
  • Marriage Certificate, where applicable
  • Adoption Register, where applicable
  • Divorce Judgment, if  applicable

Where persons who were born outside of Jamaica, have been registered as a national of Jamaica, and are applying for a passport for the first time, in addition to the requirements listed above (as applicable) one of the following forms of identification must be submitted:

  • A driver’s license (Jamaican or foreign country of residence)
  • A valid Government or State issued photo identification
  • A national voter’s identification card (Note:- All current Voters’
    ID cards with Dec. 2017 expiry date will be valid until Dec. 31, 2019)
  • A work permit from the country of residence
  • A passport from your country of citizenship or residence
  • A permanent residence identification card
  • Depending on the case, the Consular Officer may request additional documentation

If you are a Jamaican National applying for a renewal or replacement of a lost / stolen / damaged passport, please submit the following;

  • A Completed passport application form
  • Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
  • Applicants who were issued a passport as a minor must submit a valid Government photo identification.
  • The Expired Passport
  • Birth Certificate (for persons born in Jamaica)
  • Registration of Adoption (where a person has been adopted in Jamaica) 
  • Certificate of Citizenship (in the case of citizenship by descent or marriage) 
  • Naturalization Certificate (in the case of citizenship by naturalization), where applicable
  • Marriage Certificate, where applicable
  • Death Certificate, where applicable
  • Divorce Judgment, where applicable
  • A Police Complaint Report ( to be submit when a Passport is Lost or Stolen)
  • Depending on the case, the Consular Officer may request additional documentation

  2. Adults:  Replacement  — US$200.00
